چکیده

Objective: Although conservative treatment is the first choice for high cervical internal carotid artery (ICA) stenosis due to spontaneous cervical ICA dissection, surgical revascularization is necessary when the disease resists conservative treatment. Although such lesions are difficult to treat by conventional carotid endarterectomy (CEA) or carotid artery stenting (CAS), they may be treated less invasively by modified CAS with add-on devices. In this report, we present two cases of CAS performed by the combined use of balloonand self-expanding stents with a review of the literature. Case Presentations: Revascularization was performed by the same procedure in a 47-year-old man and a 43-year-old woman with left high cervical ICA stenosis. In the first case, no restenosis was noted on follow-up DSA performed after 1 year, but the self-expanding stent that initially overlapped the balloon-expandable stent had slipped off. Conclusion: High cervical carotid artery stenosis due to spontaneous cervical ICA dissection can be treated by CAS using the proximal protection technique with the combined use of a balloon-expandable stent.

[Stenting of dissected carotid arteries as a minimally invasive treatment modality].

AIM The purpose of this paper is to present our experiences with carotid artery stenting in the treatment of dissected carotid arteries, by means of self-expandable stents and selective employment of cerebral protection devices. METHODS In the period from June 1, 2006 to April 31, 2009, 6 patients with 6 dissected carotid arteries were treated with self-expandable stents (4 internal carotid a...

Endovascular stent-assisted angioplasty in the management of traumatic internal carotid artery dissections.

BACKGROUND AND PURPOSE The prognosis of traumatic dissection of the internal carotid artery is worse than for spontaneous dissections. Rapid stenting followed by antiplatelet therapy may prevent complications when anticoagulation therapy is not applicable. METHODS Patients with angiographically proven traumatic carotid artery dissection and hemodynamic significant hemispheric hypoperfusion, o...

Carotid angioplasty and stenting in a patient with high grade stenosis of Internal Carotid Artery associated with both vertebral arteries and contralateral carotid occlusion

Severe internal carotid artery (ICA) stenosis is a common cause of cerebrovascular accident (CVA) in middle-aged patients. Contralateral carotid occlusion (CCO) in patients with severe ICA stenosis is associated with high risk of CVA. Carotid endarterectomy (CEA) is associated with more complications in patients with CCO than those without CCO. In this study, we present the case of a 61-year-ol...

Primary carotid stenting.

A 66-year-old male presented with right hemispheric transient ischemic attacks. Computed tomography angiography (CTA) demonstrated a very severe atherosclerotic stenosis of the proximal right internal carotid artery, > 95% by NASCET criteria. Carotid stenting was performed using a self-expanding stent (SES) alone, without balloon angioplasty or embolic protection. An immediate anteroposterior (...

Successful interdisciplinary management of the misdeployment of two self-expanding stents into the internal carotid artery: a case report

INTRODUCTION With the widespread use of carotid artery stenting, previously unknown technical mistakes of this treatment modality are now being encountered. There are multiple strategies for the treatment of in-stent restenosis. With regard to surgical management, endarterectomy and patch plasty are favored. To the best of our knowledge, this report is the first description of a complete stent ...
